Tali relazioni sono principalmente focalizzate sugli indicatori di crescita, efficienza delle terapie ortodontiche e sulle controverse correlazioni tra occlusione dentale e postura.

    ABSTRACT
    Scientific evidence for detectable correlations between dental occlusion and body posture is still missing. Recently, several literature surveys and primary studies were performed with the aim of evaluating any possible clinical relevance of such correlations, and to ultimately propose the posturography as a useful diagnostic aid in orthodontics. Taking into account, the poor quality of current evidence with very few randomised clinical trials perfomed, being most of the studies observational without follow-up or controls, the following conclusions may be drawn: 1) Most used posturographic recordings suffer major errors in terms of repeatability; 2) Correlations between dental occlusion and body posture would reach a biological (statistical) significance, although being not clinically relevant. With current evidence, occlusal interventions aimed to treat primarily postural imbalances are not recommended.

    ABSTRACT
    Current evidence has shown that functional treatment for skeletal Class II malocclusion, irrespective of the used fixed or removable appliances, may be effective with clinically relevant skeletal effects if performed during the pubertal growth phase. While growth indicators have a primary role in the identification of the pubertal growth phase in individual patients, these indicators also suffer limitations in terms of reliability, repeatability or invasiveness. A logical combination use of several indicators in individual patients may be useful in the proper identification of the different growth phase making the treatment more efficient.

    ABSTRACT
    Le terapie funzionali delle malocclusioni scheletriche di II e III Classe rappresentano una grande opportunità in termini di riduzione della necessità di terapie ortodontiche estrattive o combinate chirurgiche-ortodontiche. Tuttavia, l’individuazione del paziente ideale per queste terapie, il corretto timing di intervento, l’uso di dispositivi progettati in modo strettamente individuale e la stabilità dei risultati rappresentano una sfida per il clinico e l’odontotecnico. L’evidenza scientifica ha dimostrato come alcuni protocolli di terapia, se effettuati correttamente, possono portare al successo terapeutico in termini occlusali, estetici e di stabilità.

    ANTLO – 1° Congresso Interregionale Abruzzo e Molise ‘Sinergie tra studio e laboratorio’

    La contrazione trasversa delle arcate dentarie costituisce una delle più frequenti cause di malocclusione. L’espansione delle arcate attuata tramite dispositivi fi ssi o rimovibili ha notevoli indicazioni, come la risoluzione degli affollamenti, intercettare l’inclusione dei canini superiori o migliorare l’estetica del sorriso. Le modalità, gli effetti e la stabilità nel lungo termine dell’espansione traversale varia in base al tipo di arcata superiore o inferiore, al dispostivi utilizzato e alla presenza di crescita. Infine, l’espansione trasversale del mascellare superiore può anche essere di tipo ortopedico e risulta utile in combinazione con il trattamento ortopedica delle malocclusioni di Classe II e di Classe III.

  • Implicazioni cliniche della biologia del movimento dentario continua a leggere

    ABSTRACT
    Il movimento dentario ortodontico è il risultato di complessi eventi biologici per molta parte ancora sconosciuti. La biologia del movimento dentario è caratterizzata da una cascata di eventi scatenati dalla compressione/tensione del legamento parodontale, che porta poi ad un processo infiammatorio alla base del rimodellamento tessutale. Tale processo infiammatorio porta con sé degli effetti collaterali come dolore o riassorbimenti radicolari. L’intensità delle forze e le modalità con cui queste vengono trasmesse ai denti, e quindi al parodonto, sono cruciali nel determinare il tipo di risposta tessutale. Anche se ad oggi non esiste un metodo che possa identificare con precisione l’intensità ottimale delle forze, è noto che forze continue danno migliori risposte biologiche con movimenti più rapidi e sicuri per il parodonto e le radici dentali. E’ anche noto che la forza dovrebbe variare in base al dente e al tipo di movimento. La ricerca sulle forze rilasciate dagli allineatori è ancora all’inizio, ma la possibilità di controllare le forze estrinsecate sul singolo dente e per fase di terapia dovrebbe essere considerata in fase di pianificazione della sequenza di terapia. Inoltre, gli effetti dei farmaci anti-infiammatori usati come analgesici durante le prime fasi della terapia ortodontica dovrebbe essere considerato anche per il loro ruolo nel limitare gli spostamenti dentari. La conoscenza dei meccanismi di base del movimento dentario è di aiuto nell’ottimizzazione delle terapie ortodontiche.

    ABSTRACT
    Aim: The success of maxillary expansion should not be assessed solely by the elimination of the teeth in crossbite; it should also be defined as the re-establishment of normal maxillary growth. The aim here was to quantify the longitudinal palatal changes in children treated for maxillary constriction associated with functional crossbite (TG) and normal controls (CG).
    Subjects and Methods: Study casts of 48 Caucasian subjects (23 TG, 25 CG; mean age, 5.2 ±0.6 years) were collected at baseline and at 6, 12, 18, 30, 42 and 54 months follow-up. The TG was treated using a cemented acrylic splint expander. Casts were scanned using a laser scanner and palatal surface area and volume, and their increments over time, were calculated. Non-parametric tests were used for the data analysis. The diagnostic performance in assessing successful treatment of palatal constriction was evaluated by the receiver operating characteristic (ROC) curves.
    Results: Significantly greater increments in palatal surface area and volume were seen for the TG group up to 30 months (p <0.05, at least). According to the ROC curves the best overall diagnostic performance in terms of accuracy was for the palatal volume at 18 months reaching up to a value of 0.85, by using a cut-off value of increments of 13.5%. Conclusions: An increase in palatal volume of at least 13.5% after 18 months out of treatment is a good indicator for the assessment of re-establishment of normal growth in subjects treated for maxillary constriction in the primary dentition.
